Expert take — Cost of Asia editorial team Bank Negara Malaysia reports medical cost inflation of 15% in 2024, and its rule requiring insurers to spread premium increases over at least three years runs only until the end of 2026. If you hold, or plan to buy, a Malaysian medical card, ask the insurer in writing what your 2027 renewal premium will be. Compare that figure with an international plan's quote, not this year's price.

Malaysia's private hospitals, among them Gleneagles Kuala Lumpur, Prince Court, Sunway Medical Centre and the Pantai group, are the reason many expats choose Kuala Lumpur or Penang. They're also where your bills land. Feather, which sells international cover, estimates a private hospital stay at MYR 450 to 1,800 a day.
You have two routes. Malaysian medical cards from local insurers cost less, but Feather notes they typically stop accepting new customers somewhere between 60 and 70. Most also cover Malaysia only. International plans cost more but travel with you, and some accept applicants into their late 70s.

1. Malaysian medical card — best for lower premiums if you're under 60
Feather names AIA Malaysia, Allianz Malaysia, Great Eastern, AXA Affin and Prudential BSN Takaful among local providers, and estimates local private premiums at MYR 2,000 to 12,000 a year. These plans are built around Malaysian private hospitals and usually issue a guarantee letter on admission.
The trade-offs are entry age, a Malaysia-only scope, and repricing. Bank Negara's December 2024 interim measures let policyholders switch to an alternative plan at the same or lower premium without new underwriting, and paused claims-driven increases for one year for over-60s on the minimum plan. Those measures were designed as temporary.
2. AXA Global Healthcare — best for joining after 70
Pacific Prime's 2026 retiree comparisons list AXA as accepting new applicants under 80, with guaranteed lifetime renewal. Evacuation and repatriation are included, and tiers run from Foundation to Prestige Plus. Pre-existing conditions are generally not covered, depending on underwriting.
3. Cigna Global — best if you have pre-existing conditions
Cigna has no stated entry-age limit in Pacific Prime's comparison. It may cover conditions such as hypertension, type 2 diabetes and arthritis after underwriting. Limits are USD 1 million (Silver), USD 2 million (Gold) or unlimited (Platinum). Evacuation is a separate module, and you'll want it if you travel in the region.
What would this move really cost you?
Use your own rent, insurance, transport and visa assumptions. Compare options before committing.
4. Allianz Care — best for evacuation and oncology on every tier
Allianz Care's Care, Care Plus and Care Pro plans include inpatient care, oncology and medical evacuation, with annual limits of USD 2.5 million and USD 4 million on the first two tiers, according to Pacific Prime. New applicants must be under 75, and renewal terms depend on the age you joined at.
5. Bupa Global — best for the highest limits
Bupa's plans run from Select to Ultimate (unlimited), with evacuation on all five. You can choose full medical underwriting or a moratorium. Under a moratorium, an old condition may become eligible after two years without symptoms or treatment. Pacific Prime lists entry under 75, with guaranteed lifetime renewal.
What to check before you decide
- MM2H: MOTAC's July 2024 criteria list health insurance among the renewal documents, and a medical check-up at a panel clinic is compulsory after approval. Ask the MM2H office which policies it accepts.
- 2027 renewal quote: ask for it in writing, especially on a local plan.
- Guarantee letters: confirm that your hospital, for example Gleneagles or Sunway, will admit you without a cash deposit.
- Age bands: premiums jump at band changes. Bank Negara's pause doesn't cover age-band increases.
- Coverage area: if you'll be treated in Singapore or at home, a Malaysia-only card won't pay.
FAQ
Is health insurance mandatory for expats in Malaysia?
There's no general requirement for every expat. MM2H participants must show health insurance at renewal, and your visa route or employer may set its own conditions.
Is local or international insurance better in Malaysia?
Local plans are cheaper while you're younger and staying in Malaysia. International plans suit people over 60, frequent travelers, and anyone who wants treatment outside Malaysia.
Why did my Malaysian medical premium go up so much?
Bank Negara links recent increases to medical cost inflation of 15% in 2024, against a global and Asia-Pacific average of 10%.
