Expert take — Cost of Asia editorial team Moratorium underwriting can quietly exclude your existing conditions for good: Allianz Care's rule only covers them after 24 continuous months with no symptoms, treatment, medication or advice. If you take a daily blood-pressure or diabetes pill, that clock may never run out. Over 60, full medical underwriting usually serves you better, because you learn up front exactly what is covered and what is excluded.
Commercial transparency: Cost of Asia has no contracted partner in this category. Providers are compared on published criteria, and no provider has paid for inclusion. If this changes, affiliate links will be labelled.
After 60, the question shifts from "which plan is cheapest?" to "which plan will still accept me, and still renew me, at 75 or 85?" Two numbers matter: the maximum age at which an insurer takes new applicants, and the maximum age to which it will renew you. They are rarely the same, and local plans in particular can close the door early.
This page is for retirees moving to, or already living in, Thailand and the wider region. The trade-off is lower local premiums against the age security of some international plans.

How we chose
- Maximum entry age and maximum renewal age
- Treatment of pre-existing conditions
- Whether the plan meets Thai retirement visa minimums
- Annual limits and evacuation
Sources are insurer documents and named broker or review summaries. We do not publish premiums. Checked on 31 October 2026.
1. Cigna Global — best for applying at 70 or older
Cigna's brochure for its Senior plan, a bundle for people aged 60 and over, states that Cigna places no age limit on applications or at renewal, and promises lifetime renewal. The bundle has a US$1 million overall annual maximum and an optional outpatient module of US$15,000. If you exclude the US, out-of-area emergency hospital care is capped at US$100,000.
The trade-off is cost over time: Cigna's product summary says premiums are not guaranteed and rise with age at renewal. Choose your deductible with that in mind.
2. Luma Hi 5 or Prime — best for Thailand residents under 70
Luma is a Thai broker; its site lists LUMA CARE CO., LTD. with broker licenses. Broker Pacific Prime's summary (updated July 2026) says Hi 5 accepts applicants up to 70 and Prime applicants under 70, with renewal guaranteed to age 99 and about 180 days a year of Thai residence required. Both are described as meeting the O-A visa minimum.
If you are already past 70, ask Luma what is still open; Thailand Starter Kit reported in September 2026 that the older Long Stay Care plan is no longer actively sold.
3. Allianz Ayudhya — best for budget cover bought before 60
Allianz Ayudhya's own plan pages for Superior Health and Care Plus set a clear age rule: buy before 60 and renew continuously, and you can renew for life; buy after 60, and renewal stops at 80. Superior Health takes applicants up to 65 on its two higher tiers, with limits of 5 or 10 million baht per confinement and deductible options from 30,000 to 200,000 baht.
The Care Plus brochure states that chronic and pre-existing conditions are not covered. That makes these plans better suited to healthy applicants than to anyone with ongoing treatment.
What would this move really cost you?
Use your own rent, insurance, transport and visa assumptions. Compare options before committing.
4. Pacific Cross — best for long-term Thai residents
Pacific Prime's August 2026 summary describes all Pacific Cross Thailand plans as lifetime renewable, and the broker notes a residency rule of six months in any 12. Neither broker summary we reviewed gives a maximum entry age, so ask for it in writing before you rely on this option.
5. Allianz Care — best for high limits if you apply before 76
Allianz Care's Care range offers plan limits of about US$1.35 million to US$5 million. Its plan pages do not state an entry age; Expatica's September 2026 review reports applicants must be 75 or under. Confirm this, and which plan family is sold where you live, before applying.
What to check before you decide
- Get both the maximum entry age and the maximum renewal age in writing.
- Choose full medical underwriting if you have ongoing conditions.
- Check the visa minimum: Thailand's O-A requires at least US$100,000 or 3 million baht of cover.
- Ask whether a health check is required at your age.
- Model premiums at age +5 and +10.
FAQ
Do I need health insurance for a Thai retirement visa?
For the O-A, yes: at least US$100,000 or 3 million baht, covering the whole stay, according to the Royal Thai Embassy in Madrid. The Thaiger reports that a Non-O retirement extension does not require insurance. The LTR Wealthy Pensioner route requires US$50,000 of cover, Thai social security, or a US$100,000 deposit, per the BOI. Rules change; check the official page before you apply.
Can I get insurance in Thailand at 75?
Yes, but options narrow. Thai local plans we reviewed close to new applicants at 65 or 70; Cigna states no maximum entry age.
Will my pre-existing conditions be covered?
It depends on underwriting. Under full medical underwriting, the insurer tells you what is excluded. Under moratorium, coverage depends on staying symptom- and treatment-free.
