Expert take — Cost of Asia editorial team In Thailand, whether you must carry health insurance depends on the visa you arrive on, not on your age: O-A holders need at least 3,000,000 baht of cover at every renewal, while retirement extensions built on a Non-O visa carry no insurance requirement. The Thaiger presents the Non-O route as the fallback for retirees who can't get qualifying cover, so the visa choice matters before you leave home. Going uninsured is still a financial risk; the point is to pick the route you can keep renewing, and insure on your own terms.

This guide is for retirees, usually 50 and over, weighing a long-term move to Thailand, Malaysia, Vietnam or Bali, the four countries Cost of Asia covers. We focus on what decides whether a retirement abroad works year after year: a visa you can renew without stress, hospitals you trust, a monthly budget that holds, and air you can breathe.

The core trade-off is visa versus cost. Thailand has the most straightforward retirement visa at moderate thresholds. Malaysia's MM2H is stable but now asks for a large deposit plus a property purchase. Vietnam's cities are among the cheapest in Numbeo's data, yet the country has no retirement visa at all. Bali sits in between, with a retirement permit for those 55 and over.

How we chose

  • A renewable retiree visa route, using embassy, government and ministry pages where we could reach them.
  • Hospital access: we name only hospitals we confirmed are operating, and say JCI-accredited only where a source states it.
  • Day-to-day cost: Numbeo's single-person monthly estimate (excluding rent) and its regional rent index.
  • Air quality: IQAir's 2025 country and city figures.
  • Practical fit: ease of daily life in English, heat, transport.

1. Chiang Mai — best overall value with a simple retiree visa

Who it suits

Retirees on a moderate pension who want a mid-sized city with a large foreign community. Chiang Mai has the lowest rent index (8.8) of the Southeast Asian cities in Numbeo's regional ranking, roughly half Bangkok's 16.3. Thailand's retirement extension requires age 50 and either ฿65,000 a month in income or ฿800,000 in a Thai bank, per the government's Thailand.go.th portal. Bangkok Hospital Chiang Mai is the local branch of the Bangkok Hospital network.

Trade-offs

Smoke. IQAir put Chiang Mai's 2025 average PM2.5 at 18.2 µg/m³, and readings passed AQI 150 in April 2026. Thailand Starter Kit describes the burning season as roughly February to April, worst in March. For older lungs and hearts, that matters.

Check before you commit

If you use the deposit route, the ฿800,000 must sit in the account at least two months before you apply and three months after approval, and never drop below ฿400,000 after that. Budget for spending March elsewhere.

2. Bangkok — best for top-tier hospital care

Who it suits

Retirees with ongoing medical needs. Bumrungrad International Hospital on Sukhumvit Soi 3 became the first hospital in Asia accredited by Joint Commission International in 2002 and runs an expatriate liaison center. Bangkok is also Thailand's main international gateway, which helps when family visits or you need to fly home.

Trade-offs

It's the most expensive base in this guide: Numbeo estimates about USD 699 a month before rent for one person, and ฿23,729 for a city-center one-bedroom (updated 31 October 2026). Heat, traffic and density wear on some retirees.

Check before you commit

Live near a BTS or MRT station so you're not dependent on taxis for appointments, and confirm your insurer bills your chosen hospital directly.

3. Hua Hin — best beach town with Bangkok access

Who it suits

Retirees who want the sea without island logistics. Bangkok Hospital Hua Hin is part of the same network as the Bangkok flagship, and the Thai retirement visa rules are identical to Chiang Mai's.

Trade-offs

Numbeo's Hua Hin estimate (about USD 586 a month before rent, city-center one-bedroom ฿15,889) rests on only 16 contributors and was last updated on 3 January 2026, so treat it as rough. The town is quieter than Phuket or Pattaya, which suits some and bores others.

Check before you commit

Ask the local hospital which specialties it covers on site and where it refers complex cases, then decide whether that distance is acceptable.

4. Penang — best English-speaking island city

Who it suits

Retirees who want English widely used, good private hospitals and a compact city. IHH Healthcare lists Gleneagles Hospital Penang as JCI and MSQH accredited; Island Hospital on Macalister Road is another major private option. Numbeo's George Town figures show a city-center one-bedroom at about RM2,067, around 18% below Kuala Lumpur, and restaurants about 28% cheaper (updated 2 October 2026).

Trade-offs

MM2H is a capital commitment. The Silver tier asks for a USD 150,000 fixed deposit (The Star, citing the tourism minister, June 2024), and MOTAC says buying a home of at least RM600,000 is mandatory after approval. The official MM2H guidelines bar selling that home for 10 years unless you upgrade. New participants can't apply for permanent residency. Numbeo's George Town data rests on just 11 contributors.

Check before you commit

Principals 50 and over are exempt from MM2H's 90-day annual stay rule, per the official guidelines. Sarawak runs its own S-MM2H with different, frequently revised terms; check its official site directly. See our MM2H guide.

5. Kuala Lumpur — best for big-city Malaysia

Who it suits

Retirees who want a capital-city lifestyle at lower cost than Bangkok. IHH lists Gleneagles Hospital Kuala Lumpur and Pantai Hospital Kuala Lumpur as JCI and MSQH accredited. Numbeo estimates about USD 594 a month before rent for one person, with a city-center one-bedroom at RM2,810 (updated 28 October 2026).

Trade-offs

Same MM2H costs as Penang. KL is hot year-round and spread out; outside the rail network, you'll rely on ride-hailing or a car.

Check before you commit

Choose a condo near both a rail station and your preferred hospital, and price the RM600,000 home requirement against actual listings before applying.

6. Da Nang — best for lowest cost by the sea

Who it suits

Active retirees on a tight budget who don't mind paperwork. Numbeo puts single-person spending at about USD 453 a month before rent, with a city-center one-bedroom around ₫14.6 million (updated 28 October 2026). Since Vietnam's 2025 administrative reform, which renamed or merged many districts and wards, Hoi An falls within Da Nang.

Trade-offs

Vietnam has no retirement visa. WhereNext notes retirees can't sponsor their own temporary residence card. Most rely on the 90-day e-visa (USD 25 single, USD 50 multiple, per Vietcetera) and leave before it expires. Citizens of the UK, France, Germany and several other European countries get 45 days visa-free under Resolution 44/NQ-CP, valid to 14 March 2028.

Check before you commit

Ask your insurer which Da Nang hospitals it bills directly and where it would send you for complex treatment. Read our Vietnam retirement visa guide for the current options.

7. Sanur, Bali — best calm Bali base

Who it suits

Retirees who love Bali but want quieter streets than Canggu or Seminyak. Bali International Hospital opened in Sanur's special health economic zone in 2025, focusing on cardiac, cancer, neurological and musculoskeletal care, per Bali.live. BIMC also runs hospitals in Kuta and Nusa Dua. Numbeo's Bali-wide estimate is about USD 563 a month before rent.

Trade-offs

The E33F retirement KITAS needs a sponsor, USD 3,000 a month in pension or income, and a bank balance of at least USD 2,000, according to Emerhub. Work isn't allowed. Emerhub says the e-visa system now uses 55 as the minimum age, but notes one listing still says 60.

Check before you commit

Confirm the age and income figures with your sponsor before filing, and see our Bali retirement visa guide.

What to check before you decide

  • Insurance rules for your exact visa, including at renewal, not just at first application.
  • Deposit timing. Thailand's seasoning rules and Malaysia's fixed-deposit withdrawal limits shape your cash flow.
  • Direct billing at the hospital nearest your home.
  • Air quality in the months you'll be there.
  • Property lock-ins. MM2H's 10-year no-sale rule ties up capital.
  • Tax residency. Ask a qualified adviser how 180 days in Thailand, or 183 days in Vietnam, affects your pension and how it's taxed at home.

FAQ

What is the cheapest place to retire in Southeast Asia?

On Numbeo's current data, Da Nang and Ho Chi Minh City have the lowest single-person costs among major cities. But Vietnam has no retirement visa, so Chiang Mai is often the cheapest place with a straightforward retiree route.

Do I need health insurance for a Thai retirement visa?

For an O-A visa, yes: at least 3,000,000 baht of cover, including at renewal. Extensions based on a Non-O visa have no insurance requirement, though going uninsured is risky.

Can MM2H lead to permanent residency?

No. Malaysia's tourism minister said in June 2024 that new MM2H participants aren't eligible to apply for PR.

Does Vietnam have a retirement visa?

No. Retirees typically use the 90-day e-visa or a visa exemption if their nationality qualifies.

What is the minimum age for Bali's retirement visa?

Emerhub reports the e-visa system uses 55, down from 60, but advises confirming before you file.
