Expert take — Cost of Asia editorial team In Bali, the clause that matters most is medical evacuation, and neither Indonesia's state scheme (BPJS Kesehatan) nor local Indonesian private plans normally include it. Serious cases are often referred to Jakarta, Singapore or Bangkok, and Feather puts an air transfer from Bali to Singapore at USD 25,000 to 80,000. Whatever else you compare, make sure evacuation is included in the plan, not just available as an add-on.

Commercial transparency: Cost of Asia has no contracted partner in this category. Providers are compared on published criteria, and no provider has paid for inclusion. If this changes, affiliate links will be labelled.

Bali handles everyday care and emergencies well. Private hospitals such as BIMC (Kuta, Nusa Dua and Ubud), Siloam Denpasar, Kasih Ibu and BaliMed see expats daily, and Prof. Ngoerah Hospital (formerly Sanglah) in Denpasar is the main public referral center. For complex surgery, cancer care or major trauma, patients are often flown off the island.

That shapes the choice. A cheap plan that pays well inside Indonesia but not for a flight to Singapore leaves the biggest bill uncovered. The other variable is age. Pacific Prime advises enrolling before 60, because entry limits and pricing get harder after that.

1. AXA Global Healthcare — best for starting cover in your 70s

Pacific Prime's September 2026 retiree guide lists AXA as accepting new applicants under 80, with guaranteed lifetime renewal. Evacuation and repatriation are standard. Tiers run from Foundation to Prestige Plus, and the broker's Bali page shows limits from USD 160,000 to USD 3.2 million for the four lower tiers. The trade-off: pre-existing conditions are generally not covered, depending on underwriting.

2. Cigna Global — best if you have existing conditions

Cigna is the only insurer in Pacific Prime's comparison with no stated entry-age limit, and pre-existing conditions can be covered after underwriting. Annual limits are USD 1 million (Silver), USD 2 million (Gold) or unlimited (Platinum), with a cheaper Close Care option at USD 500,000. Evacuation is a separate module, Evacuation and Crisis Assistance Plus. For Bali residents, it's worth adding. Pacific Prime estimates a basic Cigna plan at USD 6,500 to 7,500 a year at age 65. That's an indication, not a quote.

3. Allianz Care — best for evacuation on every tier

Allianz's Care, Care Plus and Care Pro plans include inpatient care, oncology and medical evacuation on all three tiers, according to Pacific Prime. Care carries a USD 2.5 million limit and Care Plus USD 4 million. New applicants must be under 75, and renewal depends on the age you joined at. Pre-existing conditions may be covered, subject to underwriting.

4. NOW Health WorldCare — best for chronic conditions at older ages

NOW Health accepts applicants under 80 and guarantees lifetime renewal, according to Pacific Prime. WorldCare runs from Essential (USD 3 million) to Apex (USD 4.5 million). In the broker's Vietnam guide, all tiers except Essential cover chronic conditions.

5. BPJS Kesehatan — best as the legal baseline for KITAS holders

Under Presidential Regulation 82/2018, foreigners who live or work in Indonesia for at least six months must join BPJS Kesehatan. In practice, that means KITAS and KITAP holders. Feather reports monthly contributions of IDR 42,000 to 150,000 under the old class system, which is being replaced. Care starts at an assigned Puskesmas clinic and follows a referral chain, mostly in Indonesian. BPJS doesn't pay for evacuation or treatment abroad, so pair it with private cover.

What to check before you decide

  • Evacuation wording: included or optional, the destinations covered, and who authorizes the transfer.
  • Visa rules: retirement KITAS applications require health insurance. Pacific Prime cites a minimum of IDR 25 million that includes evacuation. Confirm with your sponsor or the Directorate General of Immigration, whose official page sets the minimum age for the 5-year retiree visa at 55.
  • Direct billing: ask whether BIMC or Siloam will bill the insurer directly, or whether you'll pay upfront and claim.
  • Entry age and renewal: look for guaranteed lifetime renewal in the policy wording.
  • Scooter accidents: check exclusions for riding without a valid license or helmet.
