Expert take — Cost of Asia editorial team At Thai public hospitals, holders of a Non-Immigrant O visa (the usual retirement route) are billed in the same price group as tourists, not with foreigners who work in Thailand. That rule dates from a 2019 Ministry of Public Health price regulation reported by The Phuket News, and it mainly targets people without insurance. We could not confirm whether the groups have changed since, so if you plan to rely on public hospitals to keep costs down, ask which foreigner price group applies to your visa before you need treatment.
This page is for retirees, families and anyone with an ongoing health condition who wants a base close to good hospitals. We focus on the cities Cost of Asia covers in Thailand, Malaysia, Vietnam and Bali.
The trade-off: the cities with the deepest hospital choice (Bangkok, Kuala Lumpur) are also the busiest and, in Bangkok's case, the most expensive of our cities on Numbeo's cost index. Smaller cities score well on patient satisfaction but may not offer every specialty locally.
At a glance
| Best for | Pick | Why |
|---|---|---|
| Widest specialist choice | Bangkok | Several JCI-accredited private hospitals |
| Accredited care in a cheaper capital | Kuala Lumpur | JCI-accredited Gleneagles and Pantai hospitals |
| Retirees on a budget | Penang | High patient satisfaction, low rent |
| Smaller Thai city | Chiang Mai | Highest Numbeo Health Care Index of our cities |
| Island living | Phuket | Private network hospital on the island |
How we chose
- Presence of internationally accredited hospitals (JCI, ACHSI or Malaysia's MSQH), confirmed on hospital or group websites.
- Numbeo's Health Care Index, a survey of residents' satisfaction (higher is better). It measures perception, not clinical outcomes.
- Practical access for foreigners: international patient services and pricing rules.
Checked on 31 October 2026.
1. Bangkok — best for the widest specialist choice
Bumrungrad International Hospital on Sukhumvit Soi 3 describes itself as JCI-accredited and runs an expatriate liaison center and international referral offices. It was the first hospital in Asia to gain JCI accreditation, in 2002. Bangkok Hospital's headquarters received JCI accreditation for the sixth time in February 2023, and it runs an Expat Health Connect desk for insurance checks and appointments. Samitivej, with its main hospital on Sukhumvit Soi 49, also reports JCI accreditation.
Bangkok scores 77.3 on Numbeo's Health Care Index. The catch is cost: Bangkok has the highest Cost of Living Index (43.5) of the cities we cover. See our Bangkok hospitals guide.
2. Kuala Lumpur — best for accredited care in a cheaper capital
According to IHH Healthcare's accreditation list, Gleneagles Hospital Kuala Lumpur and Pantai Hospital Kuala Lumpur both hold JCI and MSQH accreditation. Prince Court Medical Centre holds MSQH accreditation, with ACHSI accreditation for oncology services.
Kuala Lumpur's Health Care Index is 69.5, below Bangkok's, but its Cost of Living Index (38.8) is lower too. Our Kuala Lumpur hospitals guide goes deeper.
3. Penang — best for retirees on a budget
IHH lists Gleneagles Hospital Penang as JCI- and MSQH-accredited. Island Hospital says it holds hospital-wide accreditation from both MSQH and ACHSI, and runs an international patient service. Penang Adventist Hospital was the first hospital in Malaysia to gain JCI accreditation, in 2007.
Numbeo's Penang page gives a health care score of 77.55, with "equipment for modern diagnosis" rated highest (90.18). That score rests on just 29 contributors, so treat it as a signal, not a verdict. See Penang hospitals.
4. Chiang Mai — best smaller Thai city
Chiang Mai has the highest Numbeo Health Care Index of our cities (85.1), ahead of Bangkok. Bangkok Hospital Chiang Mai is part of the BDMS group's network. Remember that the index reflects how satisfied residents are, not which complex procedures are available locally, and that the smoke season (January to April, per IQAir) is hard on anyone with respiratory conditions. See Chiang Mai hospitals.
What would this move really cost you?
Use your own rent, insurance, transport and visa assumptions. Compare options before committing.
5. Phuket — best for island living
Phuket has Bangkok Hospital Phuket, part of the same BDMS network, which also lists Bangkok Hospital Siriroj on the island. Phuket does not appear in Numbeo's Health Care Index. The island's government hospitals (Vachira, Thalang and Patong) apply the foreigner price groups described above. See Phuket hospitals.
Where coverage is thinner
Ho Chi Minh City (62.6) and Hanoi (56.9) score lower on Numbeo's index. In Bali, Numbeo's health page falls back on Indonesia-wide data, with "skill and competency of medical staff" at 46.88. BIMC Hospital runs 24-hour emergency care in Kuta and Nusa Dua and offers air and ground evacuation, and Bali International Hospital opened in the Sanur special economic zone in June 2025. Make sure your insurance includes medical evacuation.
What to check before you decide
- Confirm the hospital's current accreditation on its own site.
- Ask whether the hospital offers direct billing with your insurer.
- Check your policy's evacuation and repatriation cover.
- Ask which foreigner price group applies at public hospitals.
- Get a written health insurance quote before you move.
FAQ
Which country in Southeast Asia has the best healthcare for expats?
In our research, Thailand and Malaysia had the most JCI-accredited hospitals we could confirm in expat cities: Bangkok, Kuala Lumpur and Penang all have them.
Is Numbeo's Health Care Index reliable?
It is a satisfaction survey. Use it alongside accreditation and the specialist services you need, and check the contributor count on each city page.
Do I need medical evacuation cover in Bali?
We strongly suggest it. Numbeo's satisfaction scores for Indonesia are lower than for Thailand or Malaysia, and complex cases may need transfer.
