Expert take — Cost of Asia editorial team Malaysia's cheapest MM2H tier, a USD 32,000 deposit for applicants aged 50 and over, requires buying a home under Johor state rules directly from a developer, so it won't give you a home in Penang. For Penang, the realistic route starts at the Silver tier: a USD 150,000 deposit plus a home of at least RM600,000 that you can't sell for 10 years. Compare that with Chiang Mai's ฿800,000 deposit before you compare rents.
This page is for retirees weighing Malaysia's island state against northern Thailand's largest city. Both have large foreign communities and accredited private hospitals. The trade-off is clear: Chiang Mai is cheaper and easier to qualify for, while Penang runs in English and avoids Chiang Mai's spring smoke, at the price of a much bigger upfront commitment.

How we chose
We compared visa and capital requirements (official MM2H terms, Thailand's government portal), Numbeo rents and indices, hospital accreditations from hospital groups and a November 2026 guide, air data from IQAir and Greenpeace Malaysia, and EF EPI 2025 rankings. Checked on 4 November 2026.
1. Visa and capital
Thailand's government portal sets the retirement extension at age 50, with ฿65,000 a month in income or ฿800,000 in a Thai bank. After the first three months, the balance may fall but must stay at least ฿400,000. You renew yearly, and holders of the O-A visa need health insurance of at least USD 100,000 or ฿3,000,000.
Malaysia's MM2H Silver tier asks for a USD 150,000 fixed deposit, a RM1,000 participation fee and a home of at least RM600,000, under MOTAC's terms. You get five years and can withdraw up to half the deposit, for example toward that home. Penang may also apply its own minimum price for foreign buyers, so ask a local lawyer before you shortlist properties.
2. Cost of living
Numbeo puts a city-center one-bedroom at about ฿16,550 in Chiang Mai and RM2,067 in George Town. Penang isn't in Numbeo's regional index, but Numbeo shows George Town rents within 1% of Kuala Lumpur's. Chiang Mai's rent index (8.2) is about 30% below KL's (11.6), so by that rough chain, Chiang Mai rent is likely lower. George Town's data rests on only 11 contributors, so check live listings.
Eating out looks closer. An inexpensive meal is about ฿72.50 in Chiang Mai and RM14 in George Town, per Numbeo.
3. Healthcare
In Chiang Mai, Thailand Starter Kit's November 2026 guide lists Chiang Mai Ram Hospital and Bangkok Hospital Chiang Mai as JCI-accredited. It describes Sriphat Medical Center, on Chiang Mai University's hospital grounds, as a lower-cost option with English-speaking staff.
In Penang, IHH Healthcare lists Gleneagles Hospital Penang as JCI and MSQH accredited. Island Hospital holds MSQH and Australian ACHSI accreditation for cardiology, gastroenterology and oncology, and Pantai Hospital Penang holds MSQH.
What would this move really cost you?
Use your own rent, insurance, transport and visa assumptions. Compare options before committing.
4. Air quality
This is Chiang Mai's biggest drawback. IQAir puts its 2025 average PM2.5 at 18.2 µg/m³, an improvement on 26.4 in 2024, but its AQI was still above 150 on 16 April 2026. Thailand Starter Kit describes a February-to-April burning season, with March usually worst.
Penang isn't immune. Greenpeace Malaysia reports that a February 2025 regional haze episode raised the state's city-level PM2.5 by 10% to 26%, and that transboundary smoke hit Malaysia again in July and August. Whichever city you pick, check daily readings for the months you'd actually be there.
5. Getting around and language
Chiang Mai has no BTS or MRT. Thailand Starter Kit says city trips by Grab or Bolt usually cost ฿60 to ฿120, and the airport is 10 to 15 minutes from the center, with direct flights to Singapore, Malaysia, Japan, South Korea, China and Laos.
English is the bigger daily difference. The Star reports that Malaysia ranked 24th of 123 in the EF English Proficiency Index 2025; The Thaiger reports Thailand at 116th. In Penang, doctors, landlords and banks will usually deal with you in English.

FAQ
Is Penang or Chiang Mai cheaper for retirees?
Chiang Mai, on rent and visa capital. Numbeo's figures suggest lower rents, and its visa needs ฿800,000 rather than a USD 150,000 deposit plus a home.
Can I retire in Penang on the cheapest MM2H tier?
No. The USD 32,000 tier requires a home bought in Johor from a developer. Penang retirees start at Silver.
Where do Chiang Mai retirees go in smoke season?
Thailand Starter Kit notes that some leave for southern beaches until the smoke clears. With direct flights from Chiang Mai to Malaysia, Penang is another option to price.
